Kate Beckinsale reprises her role for the fourth installment of the franchise, which is directed by Måns Mårlind and Björn Stein. Joining the cast are Sandrine Holt, Michael Ealy, and India Eisley in new roles.

The film picks up several years after the events of Underworld: Evolution, during which Selene (Beckinsale) is captured by humans. The government and the public ultimately discover the existence of vampires and werewolves, and a full-blown war ensues. After escaping from capture, Selene sets out to end the war.
