During Thursday's "Love Turns You Upside Down" episode of the ABC medical drama, Neve Campbell makes her debut as Derek's previously unnamed fourth sister, Lizzie, when Meredith asks intern Heather (Tina Majorino) to reach out to his sisters in an attempt to get a nerve donated to help repair his hand.

While Derek (Patrick Dempsey) asks Heather to call the Shepard sisters off, Meredith (Ellen Pompeo) has other orders and the poor intern finally snaps when she's caught in the middle, ultimately telling the married couple that his siblings are out for blood. But just in the middle of her tirade, in walks Dempsey's former Scream 3 co-star.
